What to Bring to Your DOT Physical

Valid drivers license (CDL or permit). List of current prescription medications and medical history. Glasses or contacts (if applicable). A full Bladder for the urinalysis. Payment Source: cash, check, debit or credit card.

FMCSA Compliance

Dr. Finley is listed on the FMCSA National Registry of Certified Medical Examiners.  Your DOT/CDL Medical Card (MEC) will be issued immediately upon passing the exam.  And Dr. Finley will enter your longform (form 5875) information onto the FMCSA website the same day by midnight.
